RN Structural Heart Cardiothoracic Surgery Clinical Coordinator
Boulder Community Health, Boulder, CO 80303
$36.96 - $55.44 an hour - Full Time, Days, 40 hours.
Boulder Community Health is seeking an RN Structural Heart Cardiothoracic Surgery Clinical Coordinator. The Structural Heart/CTS Clinical Coordinator's responsibility is to deliver and coordinate optimal patient and family experiences from diagnosis, through evaluation, treatment, and follow up for patients with coronary and structural heart disease and is recognized as a vital core to the quality and continuum of care offered through Boulder Heart. The Cardiovascular service line encompasses a multidisciplinary team from the specialties of Cardiology, Non-invasive and Invasive, Diagnostic Cardiology, and Cardiac Surgery who the Structural Heart/CTS Clinical Coordinator will act as a liaison between in facilitating the quality initiatives.

Qualifications
• Graduate of a Registered Nursing as required by licensure
• Graduate of an Advanced Practice Nursing, or Physician Assistant as required by licensure
• Licensure appropriate to field required (RN, ANP, PA-C)
• Minimum 5 years clinical experience in a related setting.
• Program development experience preferred
• Experience in care management/care continuum coordination preferred.
Responsibilities
• The Coordinator helps to drive a quality based, patient-focused program. A coordinator is devoted to managing the program implementation and execution, patient /physician notification components, education, and outreach. The Coordinator is the primary advocate of the program, both internally and publicly.
• Triage/Referral
a. Collects and analyzes potential patient data from various channels, including echo alert data (inpatient & outpatient), cath lab data, hospital and ambulatory medical records.
b. Develops structured communication pathways to ensure identified patients' data is communicated in a timely manner.
c. Offers assistance with referral coordination for patient who meet established criteria.
d. Provides follow up to each case referred to determine appropriateness of referral, timeliness of disposition, and customer/patient satisfaction, and takes immediate action to problem-solve whenever necessary.
e. Coordinates data collection for clinician decision making related to this patient population.
f. Maintains, manages and reviews data in valve tracking data base.
g. Provides patients with information about follow-up care with cardiology.
• Liaison
a. Assist in developing and revising educational materials for the referral base related to these disease processes.
b. Provides education to individuals within the care continuum of patients with cardiothoracic surgical needs and structural heart disease.
• Functional
a. Assists in the development of annual QA plan and quarterly QA report.
b. Assists in the ongoing evaluation of service's function and goals.
c. Updates database as patient progress through their disease process.
• Hospital Required
a. Employees are expected to comply with all regulatory requirements, including Joint Commission Standards.
b. Is familiar with organization, department and job specific Environment of Care areas, including Life Safety, Utilities Management, Hazardous Materials Communications, Emergency Preparedness, Infection Control and Medical Equipment Failure. Adheres to Standard Precautions as appropriate, which may include:
c. the use of protective barriers, as appropriate (e.g., gloves, masks, gowns, pocket masks, and/or safety glasses);
d. handling and disposing of infectious waste appropriately; and
e. hand washing as appropriate
