Licensed Practical Nurse - LPNnnSanta Fe, NM ( TypennPart-timennDescriptionnnKlarus Home Healthis looking for apart time or PRN (per diem)Licensed Practical Nurses (LPN / LVN)to join our dedicated team to provide community-based care for our patients currently receivinghome healthservices in theSanta Fe, NM and surrounding areas.nnThe primary function of the LPN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.nnRequirementsnnGraduate of an accredited School of Vocational/Practical Nursing.nnCurrent State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.nnTwo years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.nnHCHB experience highly preferrednnProvide nursing care to patients of all ages in their homes under the supervision of a registered nurse.nnCoordinate care with the interdisciplinary group, patient/family, and referring agency.nnAssist in the development and implementation of patient care plans.nnMonitor patient health and report any changes to the supervising RN.nnAdminister medications and treatments as prescribed.nnEducate patients and their families on health care needs, conditions, and options.nnMaintain accurate and timely documentation of patient care and progress.nnWork in patients' homes in various conditions.nnAbility to work a flexible schedule and travel locally.nnProficiency in nursing skills as defined by generally accepted standards of practice.
