DescriptionnnPhysical Therapist:nnInitiates skilled physical therapy treatments in accordance with physician’s orders; maintains required contact with physician.nnMaintains written evaluations, progress notes and discharge summaries for residents in physical therapy program.nnConfers with other health care staff regarding special needs of residents to enhance rehabilitation or maintenance of function.nnProvides input for the Resident Care Planning meetings and attends during absences of the Director Physical Therapy.nnMaintains statistical records of residents’ treatments.nnAssists with in-service training programs upon request.nnRecords and updates resident’s plan of care in the Inter-disciplinary Kardex.nnSubmits requisition for supplies needed in the treatment of residents to Director Physical Therapy.nnPay: $34.00/hr.nnQualifications:nnGraduation from an approved school of physical therapy required.nnState license to practice physical therapy required.nnInterest in physical therapy treatment programs for the geriatric and subacute populations necessary.nnAbility to direct others necessary.nnABOUT US:nnVestraCare is a portfolio of five health care facilities in New York state, including: Brookhaven Health Care Facility in East Patchogue; Chautauqua Nursing and Rehabilitation Center in Dunkirk; Roscoe Rehabilitation and Nursing Center in Roscoe; Sunset Nursing and Rehabilitation Center in Boonville; and Susquehanna Nursing and Rehabilitation Center in Johnson City.
