Your role will include: Providing hands-on personal care and assistance with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and physical lifting when transferring participants. Assisting participants with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, and shopping. Job Requirements High School Diploma or Equivalency. Current Basic Life Support (BLS) / First Aid Certification required. Reliable means of transportation. Measuring and documenting participants' vital signs as instructed. Aiding with light meal preparation and feeding, ensuring participants are nourished and hydrated. Helping participants with safe transfer in and out of their homes, and acting as a medical escort on vehicles and to medical appointments as assigned. Assisting the Rehabilitation department by carrying out a home exercise program as indicated in the care plan developed by the IDT, after sufficient training.
