This Intake Specialist is responsible for providing referral, intake, and case management services for blind and visually impaired consumers. Intake services include gathering medical information and other documentation for the purpose of eligibility determination.
Responsibilities:
- Conduct applicant intake meetings to evaluate applicant needs and discuss program options. Determine which program is most appropriate for the applicant.
- Accurately enter referrals into the case management system and ensure timely follow-up throughout the intake process.
- Collect medical information (if applicable) and other documents to support the counselor in making an eligibility determination. Assemble and maintain electronic case files.
- Schedule application/eligibility determination appointments with the appropriate program/regional counselors once all necessary documents are received from the applicant.
Minimum and Additional Requirements A high school diploma and relevant human services or social services experience. A bachelor's degree may be substituted for the required experience.
Ability to plan, organize and coordinate work. Ability to communicate effectively. Ability to maintain records. Knowledge of community resources. Knowledge of medical and psychological aspects of disability. Knowledge of case management processes.
Ability to make decisions and work independently. Ability to establish and maintain effective working relationships with others. Responsible for meeting all designated productivity standards in a quality manner with minimal supervision.
The job operates in a professional office environment. This is largely a sedentary role; however, some filing is required. This would require the ability to move files, open filing cabinets and bend or stand as necessary.
Preferred Qualifications Experience in a public vocational rehabilitation program.
