Job Requirements

• Documentation of 2 MMRs at least 28 days apart and/or positive laboratory titer

• Hepatitis B vaccination documentation or signed declination for the unvaccinated.
• Influenza vaccine status required. Consideration for declination for religious or medical exemption only.
• 2 Negative TB skin tests or Quantiferon Gold within specified periods, updated annually.
• Documentation of 2 doses of varicella 4 weeks apart and/or positive laboratory titer
• Proof of Tdap vaccine received.
