JOB DETAILS:
Practice Setting: inpatient/outpatient
Reason for Coverage: time off coverage
Dates of Coverage: 3/24-3/30, 5/26 - 6/1, 6/18
Patient Population for Required Coverage: Adult, Geriatric
What Percentage of the Practice is Hematology %, Oncology %, Internal Medicine %: 50/50
Number of Physicians in Practice: 3
How Many APPs Provide Coverage: 2
Average Number of Patients Per Day - HOSPITAL: TBD
Clinical Area: Oncology - Inpatient Setting, Oncology - Outpatient Setting
Trauma Level/Designation: Youngstown level 3, Joes level 1, boardman non trauma
Documentation System/EMR: EPIC
Required Procedures: Chemotherapy - Intrathecal, Chemotherapy - Intracavitary - Intrapleural / Peritoneal, etc.
Required Duties: Prescribes, Orders Images, Orders Tests
Administration of Chemotherapy: Nurse oncologist
Estimated credentialing timeframe: ~120 days
Travel Reimbursement Information:

• ~BR50/night lodging cap
• All other bookings must be standard/economy
• Traveling preferred to be done outside of schedule hours

Requirement description :

• Board Certified - REQUIRED
• OH License - REQUIRED
• ACLS Certified - REQUIRED
• MUST BE WILLING TO COVER LOCATIONS AS LISTED IN JOB
• AVAILABILITY MUST BE NOTED AT TIME OF NAME CLEAR - Preference to providers that can cover ALL dates
• Flu vaccine - REQUIRED at time of credentialing
• COVID Vaccine - Preferred
• DEA - Verbal Confirmation Required at Name Clear

CERTIFICATION REQUIREMENTS :

• ACLS
• Board Certified
• DEA

STATE LICENSE REQUIREMENTS :

• Ohio

Weekend Requirements : 1:4 call weekends
