* *Patient Assessment & Treatment Planning:* Conducts evaluations, identifies therapy diagnoses, formulates treatment plans with goals, and implements therapy services for patients in various settings. Modifies care plans based on patient progress to optimize outcomes.
* *Collaboration & Documentation:* Works closely with referring providers, healthcare teams, and support staff to ensure coordinated patient care. Attends meetings, care conferences, and rounds. Maintains thorough and timely documentation following SEARHC policies.
* *Patient & Family Education:* Develops and conducts therapy programs tailored to patient and family needs. Provides clear and culturally sensitive home exercise programs, ensuring understanding through feedback and demonstrations.
* *Administrative & Operational Responsibilities:* Assists in clinic operations, maintains supplies and equipment, and ensures compliance with billing procedures for reimbursement. Stays updated with hospital directives and digital communication platforms.
* *Community & Outreach Services:* Provides therapy services in diverse settings, including hospitals, clinics, senior centers, schools, homes, and remote locations. May require travel via boat or small plane in challenging weather conditions.
* *Training & Compliance:* Educates staff on PT-related topics, conducts in-services, and participates in professional development. Adheres to APTA standards and SEARHC confidentiality and quality assurance guidelines.
* *Supervision & Resource Management:* Oversees rehab staff, manages administrative and personnel tasks, and addresses HR-related issues. Responsible for ordering supplies and assisting leadership in identifying capital equipment needs.

Job Type: Full-time

Pay: $110,000.00 - $130,000.00 per year
