*Overview*
We are seeking a dedicated *Licensed Practical Nurse (LPN/LVN)* to join our team at Crescent City. This position will involve providing high-quality care in a post-acute setting. The LPN will work within a collaborative nursing team to ensure optimal patient care, focusing on the needs of each individual resident. The successful candidate will have the opportunity to make a positive impact in the lives of our patients while working in a supportive and well-equipped facility.

*Pay Rate Travel:* $2000 per week
*Local Pay Rate: *$1840 per week

*Responsibilities*
* Administer medications and treatments as prescribed by physicians.
* Monitor patients' condition and report any changes to the supervising nurse or physician.
* Assist with the development and implementation of patient care plans.
* Provide personal care assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming.
* Document patient care and observations accurately in the PCC charting system.
* Ensure patient safety by following established protocols and procedures.
* Communicate effectively with patients, families, and team members to ensure a high standard of care.
* Assist in the orientation and training of new staff members as needed.
* Participate in quality improvement initiatives and assist with regulatory compliance.

*Qualifications*
* *Education*: Graduation from an accredited LPN/LVN program.
* *License*: Current and valid LPN/LVN license in the state of California.
* *Experience*: Minimum of 1 year of clinical experience as an LPN/LVN, preferably in a long-term care or post-acute setting.
* *Certifications*: Current Basic Life Support (BLS) certification.


* *Skills*:
* Strong communication and interpersonal skills.
* Ability to assess patient conditions and make appropriate decisions.
* Familiarity with PCC charting system or similar electronic health record systems.
* Compassionate and empathetic patient care skills.
* Ability to work independently and as part of a team.
