A human services agency in New York City is currently seeking a Licensed Practical Nurse (LPN) to join their Foster Care & Residential Health Services Department in Queens.
About the Opportunity:
- Department: Foster Care and Residential Health Services Clinic
- Schedule: Monday to Friday
- Hours: 9am to 5pm or 10am to 6pm (flex)
- Patient Population: Ages 11 to 21
Responsibilities: The Licensed Practical Nurse (LPN) will:
- Provide case /care management for all foster children
- Perform nursing assessments and appropriate referrals for dental, audiology and vision screenings on initial visit and as needed in a timely manner
- Provide daily management and coordination of appointments, supplies and medication for the foster children
- Provide counseling to youth on HIV related information, Sexual and Reproductive health, and other STD's infections
- Maintain the care plan and updates when new medical diagnosis are present for each youth
- Provide discharge planning and provide summaries for youth aging out of foster/leaving foster care of foster care
- Obtain family health history, in conjunction with caseworker, from birth parents
- Document all encounters with youth in electronic medical records
- Maintain medication record and mental health reports on youth receiving psychotropic medication.
- Provide the Psychiatrists with medical information, including laboratory results for monthly medication review.
- Provide health education to youth in the practice of preventive health and safety measures
- Perform other duties, as needed
Qualifications: - Graduate of an accredited Practical Nurse program
- NYS Licensed Practical Nurse (LPN)
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
