Job SummarynnThe Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, implementation, education, discharge planning and communication to maximize a patient's progress toward achieving their functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.nnEssential Duties and ResponsibilitiesnnSupports and promotes the mission and values of Covenant Health Ministry.nnProvides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes.nnDesigns and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.nnConducts therapy practice within defined standards of care.nnDirects the assistance of support staff to achieve patients’ functional goals.nnCommunicates effectively with other professional and support staff to achieve positive patient outcomes.nnMaintains compliance with all relevant Federal, State regulations and Organizational policies.nnFollows hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.nnDocuments evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.nnEnhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.nnProvides direction and leadership to staff and acts as a resource to promote physical therapy in the community.nnOutpatient: Schedules patients and monitors referral work queues in EHR.nnOutpatient: Monitors and analyzes essential departmental data elements through creation and running of reports in EHR.nnOther duties as consistent with this role.nnJob Requirements: (education and experience/preferred and required)nnJob Knowledge and SkillsnnClinical skills to care for the physical and developmental needs of the population age groups that will be receiving services.nnAbility to remain calm in emergency or crisis situation.nnAbility to use a computer and proficiency in EHR documentation.nnEducation and ExperiencennCurrent license from the NH state Board of Physical Therapy Examiners required.nnCPR/BLS certification required.nnGraduate of an American Physical Therapy Association (APTA) accredited physical therapy program preferred.nnAn equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.nnWorking Conditions/Physical DemandsnnMust possess the physical and mental abilities to perform the tasks normally associated with this position that involves a combination of sitting, standing, walking, bending, stooping, and reaching. Lifting/carrying frequently up to 35 pounds, and occasionally up to 50 pounds.
