Step into a key role as an RN Case Manager with our client, supporting MLTC members throughout the Bronx and Westchester, NY. This hybrid role combines remote work with in-person fieldwork, emphasizing high-quality, cost-effective care coordination and seamless service delivery.
Compensation: $105,000 - $115,000/year
Location: Based in Bronx, NY, with fieldwork across both the Bronx and Westchester, NY
Schedule: Monday to Friday (4 days remote, 1 day fieldwork)
Responsibilities as the RN Case Manager (MLTC): n

• Care Coordination:  Work closely with members and families to support independent living through regular assessments and proactive care planning.
n
• Fieldwork:  Conduct in-home reassessments one day per week, ensuring members' health and service needs are up-to-date.
n
• Authorization:  Approve covered services and coordinate care across healthcare providers and payers.
n
• Collaboration:  Maintain strong communication with members, caregivers, primary care providers, and interdisciplinary teams to optimize care outcomes.
n

nQualifications for the RN Case Manager (MLTC): n

• Education:  Bachelor of Science in Nursing (BSN) required.
n
• Licensure:  Current and active RN license with New York State registration required.
n
• Experience:  3+ years of Registered Nurse experience required (expertise in geriatrics and managed long-term care preferred).
n
• Skills:  Strong communication and advocacy abilities (Bilingual in Spanish preferred).
