*Hiring Bilingual English and Mandarin Care Manager*

We are seeking a dedicated and compassionate *Bilingual English/Mandarin* *Care Manager* to coordinate services and support for our clients and their families. This role involves working closely with caregivers and healthcare providers to ensure comprehensive care and access to essential services. The position requires a strong commitment to quality care and compliance with regulatory guidelines.

*Key Responsibilities:*
* Build relationships with caregivers and act as a liaison to other service agencies.
* Assess clients' medical, social, educational, and psychological needs.
* Develop and implement comprehensive plans of care based on identified needs.
* Coordinate and monitor service delivery and resource utilization.
* Provide at least two client-centered health home services monthly.
* Conduct monthly in-home visits and maintain accurate documentation.
* Attend ongoing staff in-services, meetings, and training.
* Respond to quality assurance feedback in a timely manner.

*Qualifications:*
* Bachelor's degree in Psychology, Sociology, Social Work, Human Services, or related field.
* Minimum of 2 years of experience in case management or medical-focused care coordination.
* Must work in-person at our Manhattan office until training completion.
* Ability to manage a full caseload of approximately 20 cases.
* Must attend weekly in-person staff meetings.
* Excellent verbal and written communication skills.
* Proficiency in MS Office (Word, Excel, Outlook) and electronic health record systems.
* Strong organizational and time-management skills.
* Willingness to travel and meet families in their homes.
* Bilingual in Mandarin/English required.
* Valid NYS ID or driver's license required.

*Physical Requirements:*
* Ability to travel throughout designated areas to meet clients and attend meetings.
* Must be able to lift and carry up to 35 lbs.
* Regular, punctual attendance required.
