Position Summary
We are currently seeking a Discharge Coordinator / Discharge Planner . The role involves continuous assessment of the patient's status and thorough documentation of pertinent information.
Essential Duties and Responsibilities
- Accurate documentation of communication and coordination for the patient's discharge and transition to the next level of care.
- The incumbent will serve as an information resource to patients and maintain consistent and effective communication with all departments within the unit.
- Responsible for matching resources and referrals to meet the needs of the patients.
Education & Experience
- LCDCi Licensed
- Licensed Chemical Dependency Counselor (LCDC).
- One year or more of experience in discharge planning or supporting patients transitional care is preferred (but not required).
