Individuals in this clinical position are primarily responsible for NLCMHA's efforts surrounding Assisted Outpatient Treatment (AOT) court orders and related activities (e.g., Deferrals, NGRI status) in coordinating with local probate courts, community partners, NLCMHA staff and inpatient mental health units. This position also coordinates with inpatient mental health units to coordinate care and facilitate transitioning individuals to community-based care. This position works with law enforcement to provide case consultation, crisis intervention, service coordination, and referrals/linkage to resources for individuals, who were identified as requiring mental health/substance use-related supports. This position reports to the Operations Manager as assigned. This position has no direct supervisory responsibilities.
*ESSENTIAL POSITION FUNCTIONS:*
* Provide letters to person served, external partners, and care plans to all courts.
* Monitor and track orders and deferral arrangements for all internal and external persons served.
* Provide oversight and support to NLCMHA AOT Monitoring efforts with community partners delivering AOT services.
* Serve as a resource, including trainings on AOT, to NLCMHA and community partners.
* Serve as a key member of NLCMHA NGRI (Not Guilty by Reason of Insanity) Oversight Committee (to be developed) and NGRI consultant to NLCMHA staff.
* Work closely with Court and Hospital Liaison surrounding AOTs and related activities.
* Serve as a liaison between inpatient mental health units and NLCMHA staff and related services to assist in transitioning persons served to community-based care.
* Coordinate inpatient psychiatric discharges for Crisis Residential Unit (CRU) admissions.
* Coordinate the continuing stay review for crisis residential services.
* Track all crisis residential admissions and discharges.
* Participate in Quality Assurance case review procedures, including customer satisfaction.
* Complete the NMRE Medicaid Weekly Report.
* Coordinate transportation to Initial Clinical Assessments (ICA), psychiatric evaluations, CRU's, etc.
* Completion of hospital discharge access screens.
* Provides direct and indirect services, including case consultation and coordination, advocacy, and referral.
* Provides mobile response to a variety of community settings upon law enforcement's request/referral, including homes, businesses, shelters, etc.
* Provide face-to-face crisis interventions to help de-escalate and/or stabilize individuals in crisis and prevent further criminal justice involvement, when possible.
* Participate actively in staff development activities including unit meetings, supervisory meetings, community meetings, applicable conferences, and review of current literature regarding services.
* Work cooperatively with Law Enforcement, Primary Care Physicians, Local Courts, Family Independence Agencies, Schools, ISD, Substance Abuse and other providers to promote provision of quality community-based services.
* Provides follow-up and support services to individuals to ensure their engagement in services.
* Complete documentation of assessments, person-centered plans, review of compliance with documentation and contract requirements, quarterly reviews, and terminal evaluation summaries within time frames prescribed by unit policy in accordance with Medicaid and/or third-party payer standards (including managed care).
* May participate in interventions, assessments and other aspects of crisis services during and after hours, as necessary.
* Performs other duties as assigned.
*LICENSING or CERTIFICATIONS:*
* Must have licensure with the State of Michigan as a licensed professional:
* Licensed Masters Social Worker
* Licensed Professional Counselor
* Psychologist
* Limited Licensed Masters Social Worker
* Limited Licensed Professional Counselor
* Limited Licensed Psychologist
* Licensed Bachelors Social Worker
* Limited Licensed Bachelor Social Worker
* Must maintain licensure
* Credentialed by NLCMHA to provide services in this professional discipline
* Must have a Qualified Mental Health Professional credential
*MINIMUM EDUCATION & EXPERIENCE:*
* Must have a Bachelors degree in social work, psychology or counseling from an accredited university.
* A licensed RN will be considered.
* Two years' experience in a mental health related field preferred.
*WORKING CONDITIONS/PHYSICAL REQUIREMENTS:*
* This position may require irregular hours.
* This position may require travel by personal vehicle if an agency vehicle is not available.
