*GENERAL SUMMARY OF POSITION:*
Northern Lakes is committed to Youth Driven Family Guided care and the resiliency of children. We are particularly committed to those infants, children and families who have experienced Trauma. This professional position provides therapeutic services to infants, children and adolescents with serious emotional disturbance and their families. Services provided include conducting assessments, making diagnoses, development of treatment plans, providing individual and family therapy using DHHS approved Evidence Based Models, monitoring of progress, and documentation of services. Additionally, this position will be involved in collaboration and consultation with other community service agencies, schools and courts. Home Based services is an intensive family service program that typically takes place in the family home or in other community-based locations. Works as part of the 6 county Child and Family Team. Primary service area is Grand Traverse and Leelanau Counties. Provides services at times and locations to meet needs of the children and families served.

*ESSENTIAL POSITION FUNCTIONS:*
* Under the direction of the program supervisor, using DHHS Evidence Based Models, provides individual therapy, family therapy, group therapy, crisis intervention, family collateral contacts, coordination of care and case management services to children birth to age 18 and their families.
* Conduct comprehensive, quality assessments of both children and families. This includes general Mental Health assessments as well as specialty assessments according to age or specific condition, e.g. Trauma Assessment.
* Reach an appropriate diagnosis for each infant/youth/child, and will develop comprehensive, quality individualized/family treatment plans based on Person Centered Planning Practices.
* Has and maintains specialized certifications as required to meet the needs of the persons served.
* Maintain agency paperwork and clinical record documentation, i.e., CAFAS/PECFAS, annual assessments, person centered plans, status reports, progress notes, and vouchers, in a timely and accurate fashion.
* Based on professional training, may be asked to provide services such as psychological testing, clinical supervision, Parent Education, community education classes or interagency presentations.
* Participate actively in staff development activities including trainings provided by the Department of Health and Human Services, and those trainings that promote skill development related to the position responsibilities.
* Work cooperatively with all community agencies and other providers to promote provision of quality, community-based services.
* Participate in quality assurance programs, and in periodic evaluation of performance and outcomes of service delivery.
* Comply with mandatory reporting requirements in situations of suspected child abuse or neglect.
* Provide crisis coverage during business hours for families receiving Home Based Services.
* Advise supervisor regarding schedule and whereabouts daily.

*SUPPLEMENTARY FUNCTIONS*:
* May represent NLCMHA on internal/external committees or work groups to enhance organizational performance and development and quality of care, including Quality Improvement activities.
* May participate in community education/community caregiver activities.
* May receive and assist in resolving complaints or inquiries related to services provided by NLCMHA.

*GENERAL PERFORMANCE RESPONSIBILITIES:*
* Set priorities and make decisions, which reflect consistency with NLCMHA’s vision and mission statements, goals, values, policies and procedures.
* Provide services and conduct tasks in accordance with terms/conditions required by the funding source, i.e., Northern Michigan Regional Entity (NMRE Contract), Michigan Department of Health and Human Services (MDHHS) CMHSP Master Contract and with applicable laws and regulations.
* Participate in approved staff development activities, in-services and supervisory sessions.
* Maintain confidentiality and develop a trusting rapport with consumers, families/guardians, staff and the general public.
* Behave and communicate in a manner that promotes a positive work atmosphere.
* Perform duties in an accurate and timely manner.
* Maintain up-to-date documentation/records/reports as applicable for position performance accountability.
* Adjust work schedule, with supervisory approval, to meet needs of children and families and/or NLCMHA needs.
* Maintain an awareness to provide a safe and healthy environment and will report all hazards and/or concerns.
* Accept other responsibilities and duties required by the supervisor, consistent with the accomplishment of NLCMHA’s goals and objectives and the functions of this position. Such responsibilities shall be incorporated into the job description if they involve a lengthy commitment of time or are ongoing.
* Advise supervisor if actual practice (activity) begins to deviate significantly from specified essential functions.

*QUALIFICATIONS & COMPETENCIES:*
Credentialed by NLCMHA to provide services in this professional discipline.

*LICENSING or CERTIFICATIONS:*
Must have and maintain licensure with the State of Michigan as a fully licensed professional: Licensed Masters Social Worker, Licensed Professional Counselor or Psychologist preferred. Limited Licensed Masters Social Worker, Limited Licensed Professional Counselor, or Master’s Limited Psychologist considered. Certification in
Michigan Department of Health and Human Service/MDHHS (formerly MDCH) Trauma Focused CBT highly preferred.

*MINIMUM EDUCATION & EXPERIENCE:*
Master's Degree in Social Work, Psychology, Licensed Professional Counseling from an accredited university. Prefer previous training and competence in Evidence Based Practices, including Trauma Focused CBT. Meets MDHHS criteria for a Child Mental Health Professional.

*KNOWLEDGE, SKILLS AND ABILITIES:*
* Specialized assessment and treatment capability to serve children with serious emotional disturbance including those with a history of Trauma.
* Basic knowledge of the Mental Health Code, and the structure, guidelines and services of NLCMHA.
* Uphold NLCMHA’s Code of Conduct and professional Code of Ethics, in addition to maintaining current appropriate State licensure/registration and standards where applicable.
* Must possess professional knowledge, consistent with license or registration and demonstrate throughout employment practice equal to acceptable professional standards.
* Ability to communicate, understand and carry out oral and written directions effectively in English and clearly present information both orally and in writing on a level suitable for public groups.
* Learn community resources and identify procedures required to use a wide range of services.
* Ability to maintain effective working relationships and productively serve as a member of a team with employees and the public plus have the ability to deal with problems courteously and tactfully.
* Works as a responsive member of a multidisciplinary team and is able to work effectively with employees of other agencies, Board members, advisory groups, committees, professional organizations and the public.
* Skill in advocating on behalf of consumers, families/guardians and NLCMHA with the community at large.
* Must have a working telephone, possess and maintain a valid Michigan Driver’s License, have a good driving record, automobile insurance coverage and have access to an automobile.
* Must be willing to transport agency consumers, including in own personal vehicle when necessary.

*WORKING CONDITIONS/PHYSICAL REQUIREMENTS:*
* This position may require irregular hours.
* This position may require travel by personal vehicle if an agency vehicle is not available.
