*EXPERIENCE:*
* At least 2 years of *Discharge Planning Experience* in a hospital acute care setting covering a variety of adult units. ER experience is a plus
* *Minimum 6 months* at a Level 1 or large academic medical center working with various inpatient units for medical discharge planning
* *Epic Charting Experience* is REQUIRED

*Certification Requirements:*
* BLS (AHA)
* RN License MS/Compact

*Summary*

As an RN Case Manager, you will be instrumental in coordinating patient care and ensuring effective discharge planning within a hospital setting. Reporting to the nursing supervisor, your core skills in managed care, patient assessment, and medical terminology will be vital in providing high-quality patient service. With premium experience in emergency medicine and critical care, you will manage complex cases and collaborate with multidisciplinary teams. Your relevant experience in both inpatient and outpatient settings will enhance patient outcomes and support the organization's mission to deliver exceptional healthcare services.

*Responsibilities*
* Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
* Coordinate discharge planning to ensure smooth transitions from hospital to home or other care settings.
* Provide patient education on health management and post-acute care options.
* Collaborate with interdisciplinary teams in emergency and acute care settings to optimize patient outcomes.
* Maintain compliance with HIPAA regulations and ensure accurate documentation of patient information.
* Utilize knowledge of medical terminology and physiology to support effective patient care.

Job Type: Contract

Pay: $2,560.00 - $2,640.00 per week
