*License*: CA OT License

*Experience:*
* 2 years of experience
* *MUST have Home Health experience*
* Must have OASIS experience
* Oasis E - after Jan 2023 experience is required
* No more than 1 year gap
* All documentation due same day
* Homecare Homebase (Productivity points, minimum average 26-28)
* That could mean 3 people or 7 people
* About 20-25 cases per week. 5-7points per day
* Absolute must - true home health experience, Oasis documentation

*Responsibilities*
* Conduct comprehensive assessments of patients' occupational needs and abilities in a home health setting.
* Develop and implement individualized care plans that address patient goals and promote independence.
* Assist patients with activities of daily living (ADLs) and provide therapeutic interventions.
* Collaborate with interdisciplinary teams to ensure effective discharge planning and continuity of care.
* Educate patients and families on home safety and adaptive strategies to enhance quality of life.
* Maintain accurate documentation of patient progress and treatment outcomes.

As a Home Health Occupational Therapist, you will be essential in delivering personalized patient care in a home setting, focusing on enhancing patients' functional abilities and overall quality of life. You will utilize your core skills in physiology, medical terminology, and anatomy to develop effective care plans. Your premium skills in assisting with activities of daily living (ADLs) and discharge planning will be crucial for patient recovery. Additionally, your experience with diverse populations, including geriatrics and individuals with disabilities, will support our mission of providing compassionate and comprehensive home health services.
