Facility: The George Washington University Hospital
Shift: Monday - Friday, 8:30 AM - 5:00 PM
Licensure/Certifications: BSN, Active DC RN License, BLS
Experience Required: Minimum 1 year of hospital case management (discharge planning) experience, Cerner experience required
Job Description:
Work with a multidisciplinary team to develop, refine, update, and communicate the plan of care for patients
Implement clinical pathways, including educational needs of patients/families
Monitor and update for deviations from the plan
