Position Overview:
We are seeking compassionate and experienced Family Nurse Practitioners to join our Primary Care teams in Dundee, Penn Yan, and Seneca Falls, New York. This is a full-time opportunity for individuals committed to providing high-quality, patient-centered care through long-term relationships. The ideal candidates will be passionate about improving the health and wellbeing of the community, providing comprehensive care to individuals and families, and being part of a supportive, physician-led organization.
Key Responsibilities:

• Provide primary care services in an outpatient setting, including routine check-ups, health screenings, and preventive care.
• Diagnose and treat common medical conditions and manage chronic diseases.
• Perform physical exams, prescribe medications, and interpret diagnostic tests.
• Develop treatment plans and educate patients on healthy lifestyle choices, disease prevention, and management.
• Collaborate with physicians and other healthcare professionals to ensure the best possible outcomes for patients.
• Maintain accurate and up-to-date patient records in the EMR system.
• Participate in ongoing professional development and continuing education.

Work Schedule:

• Full-time position, Monday through Friday.
• Standard office hours with potential for flexible scheduling.
• Light call coverage, if necessary, shared with other practitioners.

Qualifications:

• Board Certification as a Nurse Practitioner (FNP).
• New York State (NYS) Nurse Practitioner license.
• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current BLS and ACLS certifications.
• Previous experience in a primary care setting preferred, but not required.
• Strong communication skills and the ability to develop long-term relationships with patients.
• Commitment to delivering compassionate, high-quality healthcare.
• Ability to work independently and as part of a collaborative healthcare team.
