Physical Therapist - Rehabilitation Facility
Location: Antioch, CA
Join our healthcare client, a leading rehabilitation facility providing post-acute care. We are seeking a licensed Physical Therapist to deliver personalized care, enhance patient outcomes, and contribute to a collaborative healthcare environment.
Key Responsibilities:

• Develop and implement treatment plans with physician approval.
• Treat patients according to prescribed plans and monitor progress.
• Supervise Physical Therapy Assistants in accordance with state regulations.
• Assist in training Restorative Aides and support discharge planning.
• Document evaluations, progress notes, and recertifications per regulations.
• Provide training for families and caregivers to ensure smooth discharge.
• Attend care conferences, participate in staff training, and recommend medical equipment.
• Foster positive relationships with patients, families, and staff.

Qualifications:

• Valid Physical Therapist License
