    Job Requirements
  • Certified Surgical Technologist (CST) certification required
  • Primary Source Verification of professional license and/or certification within 30 days of start date
  • N95 Fit Test required within 365 days of start date
  • COVID-19 vaccination required
  • Documentation of MMR vaccination or proof of immunity required
  • TB skin test screening required within 3 months of start date
  • Health Statement signed by MD/DO/NP/PA required within 365 days of start date
  • Influenza vaccine required during flu season
  • Documentation of Hepatitis B vaccination or proof of immunity preferred
  • Documentation of Tdap vaccination preferred
    Additional Details
  • Submit a color photo taken in the past 6 months with specific guidelines
