We are seeking a dedicated and compassionate individual to provide exceptional home health services in the vibrant community of Everett, WA. This role offers the opportunity to make a significant impact on patients' lives by delivering personalized care in the comfort of their own homes.

Primary Responsibilities/Accountabilities:
* Provide services in accordance with the plan of care.
* Understand and adhere to established policies and procedures.
* Deliver nursing services, treatments, and diagnostic and preventive procedures as assigned.
* Initiate the plan of care and necessary revisions.
* Initiate appropriate preventive and rehabilitative nursing procedures.
* Prepare clinical and progress notes for each patient visit and summaries of care conferences. Submit paperwork within 48 hours to the office.
* Observe signs and symptoms and report to the physician and Clinical Director or RN Case Manager reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
* Perform clinical and progress notes or visit notes as required by assignment.
* Inform the Clinical Director or designee of changes in the condition and needs of the patient.
* Counsel the patient and family/significant others in meeting nursing and related needs.
* Participate in and present in-service programs.
* Refer patients requiring specialized skills to Physical Therapist, Speech Language Pathologist, Occupational Therapist, and Medical Social Worker.
* Submit any changes in schedule to the Director of Clinical Service/Nursing Case Manager daily.
* Participate in patient care conferences to discuss the need for involvement of other health team members.
* Observe aseptic technique as required.
* Prepare equipment and materials for treatment.
* Assist patients in learning appropriate self-care techniques.
* Participate in peer review and performance improvement as assigned.
* Participate in utilization review of medical records as assigned.
* Ensure all related documentation is kept up to date and on-site.
* Perform duties following safety standards, including the use of PPEs.
* Attend all training sessions required by the company.

Qualifications:
* Must be at least 18 years of age.
* Must have a high school diploma or the equivalent.
* Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
* Two (2) years of nursing experience preferred.

Job Type: Contract

Pay: $40.00 - $45.00 per hour
