POSITION SUMMARY - RN Case Manager
POSITION DUTIES - Discharge planning and progression
hard conversations with family, patients and open discussion with MD for plans of care.
MINIMUM REQUIRED QUALIFICATIONS - TX/compact RN license
1+ year of bed side case management experience
Post acute care knowledge
Meditech experience
LENGTH OF ASSIGNMENT - 13 weeks
SHIFT / HOURS PER WEEK - M-F 8:00a-5:00p, rotating every other weekend / 40hrs/wk
SYSTEMS - Meditech / Midas
START DATE - 1/6/2025
