POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS - 2 years' of RN experience within Case Management, BSN, AHA BLS, RN License
LENGTH OF ASSIGNMENT - 13 weeks
SHIFT / HOURS PER WEEK - Variable five 8hr shifts
40hr/week
SYSTEMS - Epic
START DATE - 1/20/2025
