POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS - 2 years current Home Health experience in Case Management and Hospice Experience
NH/compact RN license
AHA BLS
PREFERRED QUALIFICATIONS - Netsmart/Allscripts EMR experience
LENGTH OF ASSIGNMENT - 13 weeks
SHIFT / HOURS PER WEEK - 5 days per week Mon-Fri / 40hr guarantee
SYSTEMS - Netsmart (formerly Allscripts)
START DATE - 1/6/2025
