POSITION SUMMARY - RN Case Manager
POSITION DUTIES - Discharge planning- will review the patients and work towards a discharge plan, with high risk patients will do an in-depth interview and work towards a discharge plan. Will work closely with interdisciplinary team including all providers and therapies
MINIMUM REQUIRED QUALIFICATIONS - 2+ years experience in an acute inpatient setting, will consider a first time traveler if they have a good background in acute care case management
RN license
BLS
PREFERRED QUALIFICATIONS - Epic experience
CCM or ACM
LENGTH OF ASSIGNMENT - 13 weeks
SHIFT / HOURS PER WEEK - 7:30am-4pm / 40hrs per week
SYSTEMS - Epic
START DATE - 01/06/2025
