POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS - 2 years case management
Prior travel experience
Recent Epic experience
WA RN License
BLS
BSN or Associates Degree in Nursing
LENGTH OF ASSIGNMENT - 13 weeks
SHIFT / HOURS PER WEEK - 8hr days / 40hrs per week
SYSTEMS - Epic
START DATE - 1/27/2025
