Pride Health is seeking a skilled Social Worker for a 13-week assignment at a leading facility in Washington DC. This opportunity presents an amazing assignment with the added possibility of extension or securing a permanent position with a premier healthcare provider.

Key details:

• Shift : 8 Hour shift, 8a- 5p, (M-F)
• Guaranteed Hours : 40 Hours a Week
• Weekly Pay Range: $2200.00 – $2320.00

Qualifications :

• Must have a social worker license to work in a clinical setting.
• Must have BLS from AHA.
• Must have a case management experience.

Responsibilities :

• Assess patients' psychosocial needs and develop individualized care plans.
• Coordinate patient discharge planning, including arranging follow-up care and services.
• Provide emotional support and counselling to patients and their families.
• Collaborate with healthcare teams to address social, financial, and emotional barriers to care.
• Connect patients with community resources and support programs.
• Ensure proper documentation of patient interactions and care plans.
