Job Title: Social Worker – Discharge Planning

Shift: Day 5x8-Hour (08:00 AM - 04:30 PM)

Position Overview:

Pride Health is seeking an experienced Social Worker to join their team on a 13-week contract . This role focuses on discharge planning within a hospital setting, providing crucial support to patients and their families in preparing for discharge.

Key Responsibilities:

• Screen, assess, and evaluate patients and their families to identify psychosocial needs.
• Develop and implement appropriate interventions based on individual patient needs.
• Confirm discharge plans and educate, counsel, and refer as needed.
• Collaborate effectively with the multidisciplinary team and community resources.

Qualifications:

• NJ State License (must be in hand at submission; takes 6-9 weeks to obtain).
• Prior experience in a hospital setting , especially in discharge planning .
• COVID-19 vaccination (full series) + booster (no exemptions).
• EKG/Medication pass/fail test may be required before start (depending on specialty).
