Job Title: Social Worker (LMSW or LCSW-C)

Location: Baltimore, Maryland

Job Type: Contract

Duration: 13 Weeks

Shift: Day shift, 8:30 AM - 5:00 PM

Position Overview:

Pride Health is seeking a compassionate and skilled Licensed Social Worker (LMSW or LCSW-C) to join our healthcare team in Baltimore, Maryland. This is a full-time, 13-week contract position offering guaranteed hours. This is an excellent opportunity for social workers with hospital experience who are looking to make an impact in patient care and work alongside a multidisciplinary team.

Key Responsibilities:

  • Conduct high-risk screenings and readmission assessments for all patients.
  • Provide adjustment to illness counseling and perform safety assessments.
  • Collaborate with the multidisciplinary team to plan safe and effective discharge strategies.
  • Identify obstacles to discharge through proactive screening and assessment.
  • Coordinate care with Social Work and Nurse Case Management partners.
  • Provide care management coverage during absences for inpatient units.
  • Participate in initiatives focused on length of stay and throughput management.
  • Support weekend on-call duties and social work emergency preparedness operations.
  • Adhere to hospital policies, ensuring patient confidentiality and safety at all times.

Qualifications:

  • Licensure: Must hold LMSW or LCSW-C certification.
  • Experience: Prior hospital experience required.
  • Skills: Strong interpersonal and communication skills, with the ability to manage multiple priorities in a fast-paced environment.
  • Other Requirements: Ability to work collaboratively in a multidisciplinary team, providing high-quality patient care.
