*Duties and Responsibilities Summary:*
* Collaborate with the home health interdisciplinary team to assess and determine eligibility for wound care patients entering the Home Care Program.
* Conduct comprehensive wound, skin, and ostomy assessments, and develop immediate and long-term treatment plans.
* Devise individualized, resource-conscious care plans in collaboration with patients, caregivers, and healthcare teams.
* Develop and evaluate wound and ostomy management policies based on evidence-based practices.
* Provide education and counseling to patients, families, and staff on wound care, infection prevention, and treatment measures.
* Demonstrate professional conduct and maintain effective relationships with colleagues, physicians, and external entities.
* Maintain clear and concise documentation reflecting patient care and progress.
* Offer guidance and validation of wound care skills to field nurses through evaluation visits and training sessions.
* Ensure effective communication and documentation among all parties involved in patient care.
* Establish and promote protocols for best practices in wound care, and educate staff on documentation and staging techniques.
* Participate in quality improvement initiatives, data collection, and interdisciplinary meetings related to wound care.
* Ensure compliance with state and federal regulations and participate in regulatory surveys as needed.
* Conduct case load reviews, chart audits, and implement corrective interventions for identified deficiencies.
* Perform other duties as assigned, including home visits during periods of increased demand or staffing challenges.

*Role Requirements:*

*Certification(s)/License(s):*
* Current New York State Registered Nurse (RN) License.
* Certification as a Wound Care Nurse (CWCN) or Wound Ostomy Care Nurse (CWOCN).

*Education and Experience:*
* Bachelor’s Degree in Nursing required.
* A minimum of 2 years of experience in a Certified Home Health Agency or post-acute care setting.
* At least 2 years of experience in Public/Community Health and 2 years of medical/surgical experience.
* Previous experience with performance improvement (PI), including knowledge of PI tools, techniques, and program development.

*Skills and Knowledge:*
* Excellent verbal, written, organizational, and analytical skills.
* Must be licensed and registered to practice as a Professional Nurse in New York State.
