Our client is dedicated to providing compassionate, high-quality care to underserved populations. Their shelter-based healthcare program supports individuals and families experiencing homelessness, focusing on delivering holistic and inclusive care to address complex medical and social needs.
We are seeking an experienced and mission-driven Family Nurse Practitioner (FNP) to join our client’s interdisciplinary team in a shelter setting. The ideal candidate will bring clinical expertise and a passion for working with vulnerable populations, including individuals with HIV/AIDS, those with substance use disorders, and residents of shelter environments.
*Key Responsibilities:*
* Provide comprehensive primary and preventive care to shelter residents, including assessments, diagnoses, treatment plans, and follow-up care.
* Manage chronic and acute conditions, with a focus on HIV/AIDS care, substance use disorders, and co-occurring mental health conditions.
* Collaborate with shelter staff, social workers, case managers, and other healthcare providers to ensure coordinated, holistic care for residents.
* Facilitate access to specialty care, medications, and other resources as needed.
* Educate residents on health promotion, disease prevention, and self-care strategies.
* Maintain accurate and thorough documentation in accordance with organizational policies and regulatory requirements.
* Participate in quality improvement initiatives and community outreach efforts to enhance the effectiveness of shelter-based care.
*Requirements:*
* Current and unrestricted licensure as a Family Nurse Practitioner (FNP) in New York.
* At least 2 years of experience as a Nurse Practitioner in a primary care, community health, or similar setting.
* Previous experience working with populations affected by HIV/AIDS, substance use disorders, and/or homelessness is highly preferred.
* Strong understanding of trauma-informed care and harm reduction principles.
* Excellent communication, organizational, and interpersonal skills.
* Ability to work effectively in a fast-paced, resource-limited environment.

Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer._
