* Work closely with patients, families, and our interdisciplinary team to create personalized care plans
* Manage pain, symptoms, and provide emotional support to both patients and their loved ones
* Be a source of reassurance and a calming presence during difficult moments
* Empower families with the knowledge and guidance to navigate end-of-life care

*What We're Looking For:*
* Active RN license with experience in home health, hospice, or palliative care
