*Title:* Physical Therapist

*Territory:* Mid-Coast Region- Bowdoin, Topsham, Brunswick, Durham, Lisbon, Sabattus, Wales



*What You’ll Do:*


* Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association. Services may include screening, evaluation, direct service, consultation and/or instruction.
* Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources towards outcomes promoting client independence and well-being.
* Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the client’s age, condition, and needs.
* Implement client care plan activities based on treatment goals towards independent living for the client/family.





*What You’ll Bring:*


* Bachelors, Masters and/or Doctorate Degree in Physical Therapy from an accredited institution.
* Current PT license in the State of Maine.
* Current CPR card required.
* 1-year Physical Therapy experience preferred. Home health experience preferred.
* A reliable car with insurance.
