The primary purpose of the Patient Navigator position is to provide clinic and community-based HIV, STI testing, and case management services to persons with and at risk for HIV infection. Performs a biopsychosocial assessment and develops an informed care plan to address any identified social service needs and treatment adherence barriers. As part of the team, the Patient Navigator will provide services in clinical settings and on the mobile health unit including working select weekend and evening hours. Maintains the confidential integrity of all client level information.
