POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS - TN/compact RN license
2 years acute hospital case management experience
LENGTH OF ASSIGNMENT - 13 weeks
SHIFT / HOURS PER WEEK - 8:00AM - 4:30PM / 40hrs/wk
SYSTEMS - Meditech
START DATE - 12/30/2024
