Salary: $80,121.60 - $120,161.60 annually (depending on experience)

Job Description:

In this leadership role, you will be responsible for the day-to-day management of the case management program. You will work closely with the Case Management Director to ensure that all aspects of hospital-based case management are aligned with the organization's goals. Your expertise will guide a team focused on quality outcomes, effective care coordination, and resource utilization, all while maintaining compliance with hospital policies and regulations.

Key Responsibilities:

• Oversee and manage case management services for assigned area, ensuring smooth coordination and delivery of care.
• Supervise and mentor case management staff, providing guidance on clinical practices and patient care.
• Monitor the quality of interactions, assessments, and documentation by the case management team.
• Ensure optimal care delivery while balancing appropriate resource utilization.
• Promote patient-centered care in alignment with organizational goals and standards.
• Act as a subject-matter expert and resource for case management staff.

Qualifications:

• Education:
• Associate’s Degree in Nursing required
• Bachelor’s Degree in Nursing or Master’s Degree in Social Work required (or must be obtained within 2 years)

• Experience:
• Minimum of 3+ years in Acute-Care Case Management or Social Work
• Minimum of 2+ years of supervisory experience in a healthcare setting

Compensation:

• Min: $38.52/hr ($80,121.60 annually)
• Mid: $48.15/hr ($100,152 annually)
• Max: $57.77/hr ($120,161.60 annually)
