POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS - AK RN license
Previous case management experience
2 years of general nursing experience
PREFERRED QUALIFICATIONS - Bachelor's degree, BLS, National Case Management certification
LENGTH OF ASSIGNMENT - 13 weeks
SHIFT / HOURS PER WEEK - M-F / 40hrs per week
SYSTEMS - Cerner
START DATE - 1/6/2025
