Duties
The individual in this role is responsible for managing an assigned group of patients and overseeing occupational therapy services in the home setting. This includes providing direct patient care and supervising COTAs according to a plan of care that addresses patient needs, goals, and physician-ordered services.
Key responsibilities include coordinating care with a multi-disciplinary team and acting as a liaison with other healthcare providers. The role ensures timely patient care and documentation to meet both patient needs and agency goals. Additionally, this position involves directing, supervising, evaluating, and providing occupational therapy services as ordered by a physician.
The role involves planning and conducting OT programs to restore function in activities of daily living, prevent disability, and help patients achieve maximum independence. It may also involve case management, including the supervision of home health aides. The practice is guided by the Occupational Therapy Practice Act and relevant board regulations.
Essential Functions
* Manage the assigned patient caseload, ensuring timely care and documentation per the established plan of care and physician orders.
* Evaluate the patient's ability to perform daily activities, potential for improvement, and develop a care plan with the patient's physician.
* Design, develop, and fit patients with orthotic devices and adaptive equipment, providing instruction on appropriate therapy methods.
* Observe, record, and report any treatment reactions or changes in the patient's condition to the physician.
* Maintain communication with the multidisciplinary team, providing updates on schedules, patient data, and physician orders.
* Incorporate goals established in the plan of care into therapy, documented in visit notes, progress notes, and discharge summaries.
* Support effective communication and collaboration with physician oversight and branch supervision to ensure timely feedback on patient conditions or care plan changes.
* Delegate elements of care to healthcare workers in accordance with legal or policy parameters.
Skills
* Must maintain a current state driver's license and auto insurance at all times.
* Personal vehicles must be well maintained and in safe driving condition.
* 1-year experience in a clinical care setting or home health preferred.
* Basic Life Support for Healthcare Provider (BLS HCP) from AHA required.
Education
* Graduation from an accredited school of Occupational Therapy required.
* Licensed by the Examining Committee to practice Occupational Therapy in the state required.
