*Clinical Community Liaison*
*JOB SUMMARY*: Seeking a strong Master’s level clinician to join our team under the direction of the ICBS Supervisor. The Clinical Community Liaison will be assigned to a Care Management Organization (CMO) serving as a member of a multidisciplinary team to assist with navigating and understanding the collaboration between the Community Mental Health (CMH) and Juvenile Justice Systems. They will provide clinical consultation, psychoeducation, and care coordination to support adjudicated youths accessing integrated health services (physical and behavioral health services) while involved in Wayne County Juvenile Justice System. We serve youths that meet criteria for a Serious Emotional Disturbance (SED), Intellectual Developmental Disability (I/DD), or Severe Mental Illness (SMI) designation, assisting with treatment planning designed to meet their individualized needs. A Clinical Community Liaison establishes and builds rapport with youth, community partners, and case managers via face to face contact; attends home and residential visits with treatment team members for youth updates; monitors youth status promoting safety in the community or in a residential placement facility; assists with ensuring therapeutic support services that are implemented for specialized treatment toward specific goals and objectives; and facilitates multi-disciplinary treatment team meetings. The Clinical Community Liaison is also required to submit timely documentation each week for reporting service provision in the electronic database system, JAIS, to CMH Children’s Service agencies, or treatment team as assigned..
*QUALIFICATIONS*: All candidates must have a Master’s degree in a mental health related field from an accredited school; also, appropriate State of Michigan licensure is required in that professional discipline (LLMSW, LMSW, LPC, LLPC, TLLP, or LLP. Recent experience in the behavioral health/human services field is preferred, but new graduates will be considered.
Candidates must demonstrate familiarity with Medicaid policies and procedures and a general knowledge of least two (2) of the following systems are preferred: Detroit Wayne Integrated Health Network, Michigan Department of Health Human Services, Wayne County-Health, Human and Veteran Services and the Wayne County Juvenile Court system.
Must have knowledge of Systems of Care philosophy and principles. Must be a skilled observer and communicator with an understanding of integrated health and other social service systems.
